Transitional Housing
Transitional Housing allows a young person to live independently over their last months of care.
Goal:
Beliefs:
- Birth family and friends are the youth's best resources.
- Teens exiting the system need to make their own decisions.
- Experiencing consequences is vital to learning.
Referrals:
- 17 or 18 years of age
- Currently in foster or group care
- Support of:
- Foster parents or group home staff
- County Social Worker and County ILP Worker and Coordinator
Mentor:
- Non-system adult to provide support
- Selected by the young adult
Transitional Housing Team:
- Young adult and family members
- CSD Worker and ILP Worker
- Youth & Family Social Worker and Licensing Coordinator
- Friends and Mentors
